Accommodations Related to My Own Disability

If you require an accommodationA reasonable adjustment to job duties or work environment to enable an employee to perform the essential parts of his/her job without causing undue hardship related to your own disability:

  • Talk to your supervisor to see if the request can be accommodated without disclosing any confidential medical information.

  • If your supervisor is unsure about the request, or if you wish to bypass your supervisor, fill out an Accommodation Request Form and submit it (with medical certification) to HR Shared Services.
  • The Accommodations team will review your request and make an effort to grant your request. If the request is unreasonable or causes undue hardship on the department or UChicago Medicine, HR Shared Services may explore other accommodations. 
  • This interactive process may take several weeks from initial request date for a decision to be made on the accommodation.
